Hydroxyproline (Hyp) is a modified amino acid with a hydroxyl group on the fourth carbon of the pyrrolidine ring. It is a nonproteinogenic amino acid formed by prolyl hydroxylase on existing proteins, typically collagen subunits. Vitamin C (ascorbic acid) is required for the production of hydroxyproline as it keeps the hydroxylase’s iron atom in the 2+ oxidation state. Low levels of hydroxyproline in collagen are seen in patients with scurvy, while abnormally high levels of free hydroxyproline residues in serum and plasma occur as a result of Paget’s disease.
